Causes of the cigarette problem in america.?

Hey i need to know how the cigarette problem in America has gotten this bad. In other words the causes, in order. im not sure if it means tobacco growers to tobacco industries to the government to the media......OR does the question mean the start of tobacco industries in the nineteenth century to the rise of popularity of cigarettes in america to tobacco industries making new brands and different types of cigarettes...im not sure which is better or are they both wrong any out look would help and be greatly appreciated =]
